Complete Personal Profile Screen

Field Definitions

Field Name

Field Description

General

This screen contains information that is needed to complete your account set up, but was not requested when you set up your account during your online purchase. The required fields are address, daytime phone number, Key Word Question and Response, option to receive email notification for promotions/functionality changes and your email address. There are separate screens for US and International addresses.

First Name

Your First Name.

Middle Initial

Your Middle Initial.

Last Name

Your Last Name.

Address lines

The first and second lines of your address.

City

The City associated with this address.

State, Province, or Region

State in the United States for your address. You may select from a drop down list of states. Type in the province or region for an international address.

Zip Code or Postal Code:

The Zip Code for a U.S. address, or the Postal Code for an international address.

Country

For U.S. addresses the default is USA. For international addresses you may select from a drop down list of countries.

Address Description

An optional field to describe your address, (e.g., home, my place, etc.). If no address description is entered, the default is "My Address".

Daytime Phone Number

The phone number to reach you during the day.

Evening Phone Number

An optional phone number to reach you during the evening.

Fax Number

An optional phone number to contact you by fax.

Add Key Word Question

Add Key Word Response

Add a Key Word Question and its corresponding Key Word Response which will be used by Customer Service Representative for verification. Select the Key Word Question from the drop down list of questions, or create your own unique question by typing it in the field provided.

Type the answer to the Key Word Question in the Key Word Response field.

You may update your Key Word Question and Response as often as you like.

Email Address

The address at which you wish to receive electronic messages. Your email address must contain the " @" symbol and at least one period (.) after the "@" symbol.



Navigation Buttons

Button Name

Description

Add/Update Address Book

Takes you to the Address Book Summary screen from which you can add additional addresses or change or delete existing addresses in your Address Book.

Annual Date Reminder

Takes you to the Annual Date Reminders screen where you can view date reminder(s) you have established or add new date reminders. You can also make changes to an existing date reminder by clicking on the "Description" associated with that reminder which will bring you to the Update Date Reminder screen.

Change Key Word Question/Response

Takes you to the Change Security Options screen to update an existing Key Word Question or Response.

Change Password

Takes you to the Change Security Options screen to change your password.

Contact Us

Takes you to a screen containing information about how to contact Customer Service, Customer Service hours, etc.

FAQs

Takes you to a screen with a list of frequently asked questions and answers.

Help

Takes you to a screen with field definitions for the specific screen you are viewing.

Overview

Takes you to the Overview screen containing information about the service and its features.

Security

Takes you to a screen describing the security features of the service.

Sign In/Out

If not logged in, clicking on Sign In will take you to the Sign In screen. If logged in, clicking on Sign Out will take you to the Overview screen.

Update Personal Profile

Takes you to a screen that allows you to make changes to your personal profile information.

View Receipts

Takes you to a screen with a summary of all purchases made using the service, with links to detailed information, if available, for each order.
